We are surrogate family members with expertise in Geriatric Care Management. Our services are offered and provided to older adults, their children, and others involved in the older adult’s care. Our care managers work collaboratively with the older adult and the family to develop a plan of care that addresses the client’s current needs and provide recommendations for future care.
